remove unused ETL mutex and thread classes
    
    Smach class just fakes that it is using them (by using mutex_null)
    
    Resource class uses it, but it can be replaced by C++11 standard mutex.
    
    That ETL_LOCK_REFCOUNTS directive is not mentioned anywhere.
    Maybe it should be removed?
